Are the back to back diodes going directly to a ground trace or staying strictly on C6's leads?? Also, do you have any acetone??
Unsolder and lift one leg of D9. Power back up and see if you still have 40v+dc . Also check rail votage. I want to see just how high it goes with the DBT you have in line...

Are you taking the variac to 100%.
Are Q5, Q7, Q10 getting HOT??
Take the dbt out of circuit and briing the variac up to 80 volts. Then see what you have bias and DC offset.
